  • Under Section 2799B-6 of the Public Health Service Act, health care providers are required to provide patients who are uninsured or who are not using insurance with a Good Faith Estimate of expected charges for items and services. You have the right to receive a Good Faith Estimate for the total expected cost of any non-emergency health care services.

    You can ask your health care provider for a Good Faith Estimate before you schedule a service. If you receive a bill that is at least $400 more than your Good Faith Estimate, you can dispute the bill.   • All services provided by Dr. Campbell are confidential and governed by California law, HIPAA, and the ethical standards of the American Psychological Association (APA). Information shared during evaluation or therapy will not be disclosed without your written consent, except as required or permitted by law (e.g., imminent danger to self or others, suspected child or elder abuse, court orders).

  • Telehealth services are provided in compliance with California Business and Professions Code §2290.5. By participating in telehealth, you acknowledge that telehealth involves the delivery of health care services using electronic communications, and that there are potential risks including, but not limited to, interruptions, unauthorized access, and technical difficulties. You retain the right to withdraw consent and discontinue telehealth at any time.

  • All clients will receive and sign a detailed informed consent document before services begin. This document outlines the nature of services, fees, cancellation policies, confidentiality and its limits, and your rights as a client.
